Member Request Form: Chicago Elders

The Chicago UBF Board of Elders asks members to use the Member Request Form for communication of requests and reporting issues to elders. This form will help to streamline and centralize all of our member requests, assist in tracking issues and result in more timely responses to you. Please use this form instead of sending direct emails and texts to elders. We look forward to serving each of our Chicago UBF members.
